Ride Home Acknowledgment and Agreement

At Soft Reboot Wellness, the safety of our patients and the community is our highest priority. Due to the effects of ketamine therapy, we require that all patients arrange for a responsible adult to drive them home after their treatment.

Acknowledgment of Responsibility:

I, _____________________________ (Patient’s Name – Type or Print), understand and agree to the following:

1. I will arrange a ride home with a responsible adult after my ketamine treatment. This ride may include a trusted family member, friend, or a professional service such as Uber or Lyft.
2. I acknowledge that the half-life of ketamine is 3-4 hours, and I understand that my ability to drive or operate heavy machinery may be impaired for up to 12 hours after the treatment.
3. I will wait at least 12 hours after the completion of my ketamine treatment before:
   • Driving or operating heavy machinery.
   • Signing legal documents.
   • Making important decisions.
   • Working or engaging in any tasks that require clear cognition, control of my physical body, and reflexes.
4. I understand that if it is discovered that I have driven myself home after ketamine treatment, I will be terminated as a client. This is because driving under the influence of ketamine poses a serious danger to myself and others.

Consequences of Non-Compliance:

• Driving after receiving ketamine treatment is a violation of the policies of Soft Reboot Wellness and can result in immediate termination as a client.
• I understand that by choosing to drive after ketamine treatment, I would be putting myself and others at significant risk, which is unacceptable.
